For players who fell into the bottomless gameplay loop of the original Vampire Survivors, the core DNA remains firmly intact. Weapon evolutions, treasure chests containing multiple items, mysterious arcana cards, challenging modifiers, and a robust progression system are all present and accounted for, though thoroughly adapted to fit the Warhammer aesthetic. Experience gems, for instance, have been appropriately swapped out for experience skulls. Meanwhile, the terrifying Red Death—the unavoidable, run-ending reaper that traditionally arrives to put players out of their misery in Vampire Survivors—is replaced here by intimidating "Extremis" bosses. These towering, seemingly invincible foes demand that players learn specific tactical counters rather than simply trying to outrun them.

The game’s maps are similarly steeped in faction-specific lore, offering drastically different visual identities and environmental hazards depending on the chosen universe. In the Warhammer 40,000 side of the experience, players will find themselves sprinting across terrain being actively consumed by Tyranids, complete with aggressive pink palettes, grotesque alien topography, and pulsating pustules. The Age of Sigmar component shifts the focus toward the subterranean filth of the Skaven, featuring glowing warpstones and skittering hordes. In a particularly clever nod to the hobbyist roots of the franchise, one specialized map takes place directly on a gaming tabletop, surrounded by paint pots and measuring tapes, where miniature monstrosities gather to overwhelm the player.

Battles escalate at a much faster pace, demanding more precise positioning and careful experimentation with weapon combinations to successfully thin out the encroaching hordes. Healing resources feel scarcer, forcing a higher level of engagement during the opening minutes of a run. While some of this early-game intensity might stem from the limited roster of characters and weapons available in the demo build, it is clear that surviving the grimdark future will require sharper reflexes.

Players select their preferred setting before embarking on a run, with characters, upgrades, and unlocks remaining strictly partitioned within their respective universe, though certain passive abilities cross over. This design choice makes the final product feel like two distinct, content-rich spin-offs packaged into a single release.
